* Felines you are interested in may or may not have received a rabies vaccination. Without a veterinarian on staff (or if that animal has no reason to undergo a spay/neuter or any other surgery) it is impossible to secure enough veterinarian appointments to update rabies vaccinations. If the feline you are filling out an adption application for does NOT have one, this will be disclosed to you. You would take responsibility for vaccination by working with your own veterinarian after adoption.

Cats
Kittens (under 10 months) - $175
Adults (10 months to 4 years) - $125
Senior (5 years+) - $55


Dogs
Puppy (under 10 months) - $350
Adults (10 months to 8 years) - $250
Senior (8 years+) - $175


Small Animals
Rabbits - $45
Guinea Pigs - $25

  • Certify that I am at least 18 years of age and the information given is true. I authorize any and all verifications of statements made in the application. I recognize that any misrepresentation of facts may result in losing the privilege of adopting a pet.

  • Understand vaccines, vet care and proper nutrition can cost $300-500 per year and I agree to financially provide for this animal during its entire lifespan which may be 20 years or more.

  • Understand SWCHS has the right to do a follow up visit and take pictures showing proper shelter is being provided for any animal kept outside. If this animal goes missing I will contact the SWCHS immediately.

  • Understand how the adoption process works, agree to complete any spay/neuter requirements and pay fees involved with adoption.

  • Understand all terms and conditions.
